We try to get appointments arranged as quickly as possible

MD Appointments

• If you would like to schedule an appointment, please call our office @ (530) 343-8438, and any of our receptionists would be happy to assist you.
• We will send you a medical questionnaire prior to your scheduled appointment. Please complete this questionnaire and bring it with you to your appointment. The information you provide Dr. Egert is a valuable part of his evaluation.
• If you do not have this paperwork completed, please arrive 20 to 30 minutes earlier than your appointed time.

MD Cancellations

• We request 48 hours notice for cancellation of an appointment.
• You may leave a message with our voice mail system during times when the office is closed. If you are ill and unable to keep your appointment, please call to reschedule as soon as possible.
• There is a missed appointment fee for cancellations with less than 48 hours notice. Missed appointment fees are your personal responsibility and cannot be billed to your insurance company. We will expect the missed appointment fee to be paid at your next scheduled appointment

PT Appointments

• If physical therapy has been prescribed for you by your physician, or by Dr Egert, please call our PT receptionist @ (530) 343-8438 to arrange your treatment sessions. For more information on what to wear, etc. please click on Physical Therapy Sessions
• In most cases, a prescription from your physician is required in order to schedule an appointment for physical therapy. If you do not have a prescription, you can request to be evaluated by Dr Egert, our physiatrist who may order physical therapy if such treatment is appropriate for your condition.

PT Cancellations

• We request 24 hours notice for cancellation of a physical therapy appointment. You may leave a message with our voice mail system during times when the office is closed. Please call to reschedule your appointment as soon as possible to avoid disrupting your treatment program.
• There is a $25 missed appointment fee for cancellations with less than 24 hours notice. Missed appointment fees are your personal responsibility and cannot be billed to your insurance company. We will expect the appointment fee to be paid at your next scheduled treatment session.

Massage Therapy

• Jessica Birdseye, CMT, is our Massage Therapist. Massages, as part of our physical rehabilitation program, must be pre-authorized by your insurance company.
• If your insurance policy does not cover massage, you may wish to contact Jessica directly at (530) 864-1659 to arrange for an appointment, on a private, cash-pay basis.
• Cancellations, rescheduling, and payment for private massage appointments should be done through Jessica. Our staff does not have information regarding Jessica's private patients, so we ask that all communication regarding private massages be done through directly with Jessica.
