As a courtesy, we will bill your insurance company. Please provide the front office with your
complete insurance billing information on your first visit. Your insurance company may require
pre-authorization before physical therapy services are initiated.
Please verify your benefits with your insurance company for possible limitations in coverage.
We will attempt to collect payment from your insurance carrier. However, all charges are
ultimately your responsibility.

If you have billing questions regarding your Workers' Compensation claim, you should contact
