Do I need a referral to schedule an appointment with Robert Egert MD Physical
Rehabilitation?
- NO. As a Physiatrist, Dr. Egert's area of expertise is Physical Medicine and Rehabilitation.
Patients are often referred to him by other physicians for specialized diagnostic procedures and
treatment. However, we also welcome the opportunity to assist you at the onset of your injury or
pain. For many of our patients, scheduling an appointment with Dr. Egert is their first step
toward early diagnosis and recovery.
May I have Physical Therapy at your facility even if I'm NOT a patient of Dr. Egert's?
- YES. If your physician has prescribed Physical Therapy treatment for you, our Physical
Therapists provide treatment according to the orders written by your doctor. We have an extensive array of sophisticated equipment
- Unlike other outpatient physical therapy programs, our therapists benefit from having an on-site
medical director Robert Egert, MD, a Board Certified Specialist in Physical Medicine and Rehabilitation. About my medical speciality
- Physicians will sometimes send you to Physical Therapy for 4-6 weeks. If your symptoms
persist, your doctor may refer you to Dr. Egert for a consultation and possible treatment.PMR
Will you bill my Insurance?
- YES. We accept most insurances, including Medicare and the Blue Cross network.
As a courtesy, we will bill your insurance company.

Can I get a copy of my Medical Records?
- Yes, at your request we will provide you with a copy of your medical records pertaining to
treatment received at our facility. There is no charge for the first copy. Additional requests for
records will need to be processed through a copy service.

What if I need forms completed by Dr. Egert? more information
- If you have forms requiring Dr. Egert's signature, you may submit them to our office with
instructions as to where you'd like them mailed, or if you'd like a phone call to pick them up
when they are ready.
How do I get the information I need to submit my mileage report to my insurance
company?
- Upon request our receptionists can provide you with a list of dates you were seen at our office for
treatment.
What if my rehabilitation plan includes Vocational Rehabilitation?
- If your vocational rehab counselor has given you forms to be completed by your doctor, we will
need to obtain authorization to process the forms from the insurance company. Please be aware
that the authorization request and subsequent review of your records may require a considerable
amount of time.
