Medical Records
At your request we will provide you with a copy of your medical records pertaining to treatment
received at our facility. There is no charge for the first copy. Additional requests for records will
need to be processed through a copy service.
If you would like a copy of your medical records, please send a request in writing to:

If you have been referred to another physician, and we have a signed "Release of Records" on
file we will fax the records directly to the other physician's office.
Mileage Reports
Upon request our receptionists can provide you with a list of dates you were seen at our office for
treatment.
Vocational Rehabilitation
If your vocational rehabilitation counselor has given you forms to be completed by your doctor,
we will need to obtain authorization to process the forms from the insurance company. Please
be aware that the authorization request and subsequent review of your records may require a
considerable amount of time.
Disability Forms
If you are currently receiving treatment from Dr. Egert for an injury or ailment that has resulted
in work restrictions, etc., Dr Egert will typically provide you with a notice for you to submit to
your employer. Concerns regarding your restrictions should be addressed at the time of your
appointment. If you have forms requiring Dr. Egert's signature, you may submit them to our
office with instructions as to where you'd like them mailed, or if you'd like a phone call to pick
them up when they are ready. Please allow 5-7 days for this process.
